You can start the process of establishing care by scheduling an intake with one of our providers below.  After you schedule an appointment, you will be emailed information on how to set up a patient portal with our electronic medical record, ChARM, in addition to several intake forms with personal demographic and health history information.  We request that you complete these forms and submit them for your provider to review through the ChARM patient portal.
