We see patients aged 18 and older. We do not perform neuropsychological evaluations.  

One of our physicians will contact you if they are able to see you; there is no need for you to call or email after completing this form. Please note that completing this form does not guarantee an appointment or a call back. We delete inquiries after 90 days whether or not we have been able to get you an appointment. Submitting this form does not establish a doctor-patient relationship.

Please make sure to check with your insurance company about the details of your mental health care coverage (ensuring the doctor is in network, deductible, co-pay, co-insurance, etc). We do not contact insurance companies for preauthorization or verification of benefits. 

For urgent psychiatric care, please contact your county mental health crisis center (Cascadia Behavioral Health in Multnomah County, OR and Hawthorn Clinic in Washington County, OR), your primary care physician or provider, go to an emergency room or Unity Center for Behavioral Health in Portland, OR, or call 911.
